Explanation Of Services
Agreed upon duties vary depending on individual circumstance, the following are examples of some common types of support
-Meal preparation and feeding as needed
-Bathing and toileting hygiene
-Personal care and grooming
-Assistance with ADLs (mouth care, dressing, range of motion exercises, etc)
-Communication of needs/changes of client with family caregiver
-Offer family educational resources/support as needed
-Light housekeeping/maintenance of household throughout care
-Bedside care for minor temporary illness
-Enable family caregiver to run errands and buy groceries by providing companionship and
safety supervision of the client
-Provide family caregiver with reminders of responsibilities in care (ie: medication administration in line with providers scheduled orders, follow up calls to MD for information, what supplies are needed, etc)
-Maintain consistent communication about client status with family caregiver, relaying all
essential information to be communicated to medical team

Additional support needs are assessed on a case by case basis, through our intake and consultation process- in order to form a personalized contract.
